Liability Waiver, Medical Authorization & Media Release

Please read this agreement carefully before registering for the Ukrainian Young Talent Showcase.

By registering for this event, the participant (or the participant's parent/legal guardian if under 18 years of age) acknowledges and agrees to the following terms and conditions.

1. Assumption of Risk

Participation in soccer involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, weather conditions, and other unforeseen circumstances that may result in serious injury, permanent disability, or death. I voluntarily assume all risks associated with participation in the Ukrainian Young Talent Showcase.

2. Release of Liability

I hereby release and forever discharge the Ukrainian Diaspora Sport Association (UDSA), Ukrainian Young Talent Showcase, Chicago Fire Pitch, event organizers, directors, employees, volunteers, coaches, scouts, referees, sponsors, agents, representatives, and affiliates from any and all liability, claims, demands, damages, actions, or causes of action arising out of participation in this event.

3. Medical Authorization

I certify that the participant is physically fit and medically able to participate.

In the event of an emergency, I authorize the event organizers to obtain emergency medical treatment if necessary.

I understand that I am responsible for any medical expenses incurred.

4. Media Release

I grant permission to the Ukrainian Diaspora Sport Association (UDSA) to photograph, record, livestream, and otherwise capture images, video, and audio of the participant during the Showcase.

I authorize UDSA to use these materials for promotional, educational, marketing, social media, website, and future event purposes without compensation.

5. Player Evaluation

I understand that participation in the Showcase provides an opportunity to be evaluated by professional scouts, college coaches, and football agencies.

Participation does not guarantee:

  • selection by any club

  • representation by any agency

  • a college scholarship

  • an invitation to trials

  • a professional contract

All evaluations and decisions remain solely at the discretion of attending scouts, clubs, colleges, and agencies.

6. Refund Policy

Registration fees are non-refundable, except in the event that the Showcase is canceled by the organizers.

7. Code of Conduct

Participants are expected to demonstrate professionalism, respect, and sportsmanship at all times.

The organizers reserve the right to remove any participant whose behavior is deemed unsafe, abusive, discriminatory, or detrimental to the event without refund.

8. Privacy

Personal information collected during registration will be used solely for event administration and communication with participants.

Player information may be shared with attending scouts, college coaches, football clubs, and professional agencies for evaluation purposes.

UDSA will not sell participant information to third parties.

9. Agreement

By submitting the registration form and completing payment, I acknowledge that:

  • I have carefully read this agreement.

  • I fully understand its contents.

  • I voluntarily agree to all terms.

  • If the participant is under 18 years of age, I certify that I am the participant's parent or legal guardian and have the legal authority to accept these terms.
